Your Medical Records – Your Choice

You may have received a leaflet in the post from the NHS called 'Better information means better care' and have questions about how your medical records information is used.

The NHS has asked for your consent to share your information about the care you receive with those who plan health and social care services, as well as with approved researchers and organisations. You have the right to opt out of sharing this information. If you do not opt out it will imply you have given your consent for these data to be shared.

The practice also needs your consent to share your data with other health care professionals engaged in looking after your immediate health needs who use the same computer system as the Surgery, such as the district nurses, local hospitals and other community services. This is quite separate from the NHS care data consent. Please ask at reception for the relevant consent form.

Surgery Reception is open Monday to Friday - 8:00am - 6:30pm. You can also book evening appointments between 6:30pm and 7:00pm.
The surgery is closed at all other times including Bank Holidays. There is a ramp and wide access. All consultation rooms are on the ground floor, with a Cabin at the rear for the Nurse Practitioner.
